Prior to joining DSI Renal, Inc., Mr. Goguen was the Chief Operating Officer of Odyssey Healthcare, a $720 million provider of hospice services in the United States. Mr. Goguen was responsible for turnarounds in clinical outcomes, revenue growth and profitability. At Odyssey Healthcare, Mr. Goguen grew EBITDA from $24M million to $95 million in three years.
Mr. Goguen has had extensive experience in the dialysis industry where he held senior positions at DaVita, Inc., a $5 billion provider of dialysis services in the United States. At DaVita, Mr. Goguen held positions as Director of Operations and Business Development and later as Vice President of both the Northeast and Southeast divisions.
Mr. Goguen has an MBA from Harvard University Graduate School of Business Administration and a Bachelor of Arts degree from Amherst College. |

Prior to joining DSI Renal, Inc. as General Counsel, Mr. Bickham served for seven years as the Senior Vice President and General Counsel for Odyssey HealthCare, Inc., the largest provider of hospice services in the United States. Prior to Odyssey HealthCare, Mr. Bickham was in private practice at Vinson & Elkins LLP where he represented health care providers, including hospitals, ambulatory surgery centers, physician group practices, skilled nursing facilities, and home health and hospice agencies in a variety of matters including mergers and acquisitions, public offerings, government investigations and other regulatory matters. Prior to attending law school, Mr. Bickham was a certified public accountant in private practice for seven years.

Mr. Cummings has extensive IT experience in the dialysis and healthcare industry where he has held IT related positions at IMRAC and National Nephrology Associates (NNA). Before joining DSI Renal, Inc., Mr. Cummings was the Director of Data Center and Network Services for Renal Care Group, a $4 billion provider of dialysis services for patients with end stage renal disease, where he was responsible for all of the primary and backup data center operations serving over 400 dialysis medical facilities in 33 states.
Mr. Cummings has a BS in Electrical Engineering from Western Kentucky University and is a member of the Institute of Electrical and Electronics Engineers (IEEE).

Ms. DeBlois has been in healthcare since 1982 joining the dialysis industry in 1989. Her experience is in revenue, business and information technology management. Her extensive background in revenue management includes dialysis and nephrology services. Her experience in information technology includes systems management, development and implementation of various dialysis billing and clinical systems and nephrology electronic medical records. Prior to joining DSI Renal Inc., Ms. DeBlois held management positions with RenalWest, Renal Care Group, Southwest Kidney LLC, Fresenius Medical Services and Liberty Dialysis.

Johnie Flotte

Prior to becoming Vice President of Education and Quality at DSI Renal, Inc., Ms. Flotte served as Director of Education, Regional Educator, and Clinic Manager at DSI Renal, Inc. and has worked in the dialysis industry for more than 12 years. Her operations experience also includes dialysis regulations and emergency preparedness. Prior to joining DSI Renal, Inc., Ms. Flotte worked with Fresenius Medical Care in an acute program based in Louisiana. Ms. Flotte also has critical care experience, starting her nursing career at Our Lady of the Lake Hospital in Baton Rouge, Louisiana.
Ms. Flotte was awarded the ANNA Excellence in Nephrology Nursing Management Award in 2008 for her dedication and extensive contributions during Hurricane Katrina. Ms. Flotte graduated in 1994 with an Associate's Degree in Nursing from Our Lady of the Lake College of Nursing and Allied Health and holds licensure in multiple states. She is a member of ANNA and NRAA. |

Jason Gunter

Mr. Gunter has extensive experience in the health care industry with a specific focus in the dialysis industry where he has held senior finance positions at Renal Advantage, Renal Care Group (RCG) and National Nephrology Associates (NNA). At RCG, Mr. Gunter was responsible for the management and oversight of the Revenue Cycle process which represented $1.5 billion in annual revenue. At NNA, Mr. Gunter's responsibilities included Revenue Cycle, preparing annual operating budgets and negotiating managed care contracts.
Mr. Gunter received his Bachelor of Science degree from the University of Alabama where he majored in Accounting. He passed the Certified Public Accounting exam in 1995.

M. Stephen Harrison

Mr. Harrison is a 35 year veteran of the healthcare services industry, in which he spearheaded the development of over 200 alternate site therapy locations, mostly in the areas of chronic and acute dialysis. His responsibilities included medical director negotiation, site selection, construction management, provider-payer relations, and licensing and regulatory.
He was responsible for founding Community Dialysis Clinics, which ultimately was transformed into Vivra, a New York Stock Exchange company. While at Vivra, the company grew from 4 dialysis clinics to 36 clinics. He later headed new business development at REN Corporation USA, where he grew the company from 15 dialysis clinics to over 70 clinics. In 1998 he co-founded National Nephrology Associates which he grew from startup, with no dialysis clinics, to 85 clinics.

Dr. Mark R. Kaplan

Dr. Kaplan graduated from the University of Pennsylvania with a Bachelor of Science degree in Bioengineering. He completed medical school and a residency in Internal Medicine at Vanderbilt University, followed by a year as Chief Resident in Internal Medicine at the Nashville Veterans Administration Hospital. From there, he completed a clinical and research fellowship in Nephrology and Hypertension at Harvard University Medical School's Brigham and Women's Hospital in Boston, Massachusetts. Since 1997, Dr. Kaplan has been in practice as a partner with Nephrology Associates in Nashville, Tennessee. He launched and developed the practice's clinical research program. Prior to joining DSI, he held the position of Vice President of Clinical Research for Fresenius Medical Care.

As Chief Administrative Officer, Ms. Lefkovitz is responsible for information services/technology, compliance and purchasing. Ms. Lefkovitz has extensive experience in business operations and information technology. Prior to joining DSI Renal, Inc., she was EVP for IMRAC Corporation, an IT affiliate of National Nephrology Associates (NNA). At IMRAC she was responsible for operations, customer service, contract billing and collections services, HIPAA compliance and development and implementation of software for use ranging from in physician practices to statewide governmental institutions. Additionally, she was responsible for all information technology, which included a fully integrated online electronic medical record and billing system, at NNA's centralized corporate office and its 85 dialysis clinics.
Ms. Lefkovitz has a Master of Science degree in Mathematics from The University of Tennessee and a Bachelor of Science degree from Tulane University.

As Vice President with responsibility for human resources and payroll, Ms. Stewart brings more that 20 years of healthcare experience to this position. Prior to joining DSI Renal, Inc., in 2008 she held similar positions with in a wide range of health care organization including a hospital company, imaging centers and a physician practice management company.
Ms. Stewart holds a Masters of Public Administration degree and Bachelor of Arts degree from Valdosta State University. |
Debbie Taylor

Prior to joining DSI Renal, Inc., Ms. Taylor was Regional Vice President of the Middle Tennessee Region at Fresenius Medical Care. Ms. Taylor was responsible for clinical and financial operations. Ms. Taylor has extensive experience in the management of dialysis operations with more than 22 years experience.
Ms. Taylor received her nursing degree in 1984 and her Masters degree from Vanderbilt University in Health Systems Management in 2004. |
Giles Ward

Mr. Ward has broad healthcare experience with a recurring focus on financial and operational analytics, modeling, and reporting. Prior to joining DSI Renal, Inc., Mr. Ward was part of the Healthcare Consulting Group at Lattimore Black Morgan & Cain (LBMC) where he worked on the successful merger and FTC defense of two not for profit Health Systems in Tennessee. At LBMC, Mr. Ward also worked on engagements that included strategic planning for hospital clients, Certificate of Need (CON) opposition and defense, and due diligence for numerous acquisitions, divestitures, mergers, and investments in the healthcare market. He started his healthcare career at WebMD which became Emdeon Business Services.
Mr. Ward earned an MBA from Vanderbilt University and Bachelor of Arts degrees from the University of North Carolina at Chapel Hill.

Prior to joining DSI Renal, Inc., Ms. Workman was a Senior Manager in the Assurance Services practice of Ernst & Young LLP. While at Ernst & Young, Ms. Workman specialized in the health sciences industry serving dialysis industry clients including Renal Care Group and clients providing pharmacy benefits management services, acute care, hospice care, and psychiatric care.
Ms. Workman received a B.A. in accounting from Middle Tennessee State University and is a Certified Public Accountant. Ms. Workman serves on the board of Leadership Healthcare and the finance committee of Alive Hospice.

Prior to joining DSI Renal, Inc. as General Counsel, Mr. Yalowitz was in private practice in Chicago for almost 15 years, as a partner with McGuireWoods and prior to that, Ross & Hardies. Mr. Yalowitz represented health care providers including dialysis facilities, hospitals, ambulatory surgery centers, physician practices and ancillary providers in a wide range of matters including mergers and acquisitions, regulatory matters and corporate matters.
Mr. Yalowitz received his undergraduate degree from the University of Michigan and his law degree from Washington University in St. Louis.
