Jerome S. Tannenbaum, M.D.,Ph.D., FACP
Chairman, President and Chief Executive Officer

Dr. Tannenbaum holds an M.D. from Vanderbilt University School of Medicine and a Ph.D. in Pharmacology from Vanderbilt University School of Graduate Studies. He completed his training in Internal Medicine at the Jewish Hospital of St. Louis and his Nephrology training at Barnes Hospital, both affiliates of Washington University of St. Louis School of Medicine.

He is Board Certified in Internal Medicine and Nephrology and is a Fellow of the American College of Physicians (FACP).

He was the founder of REN Corporation-USA where he served as the President, Chairman and Chief Executive Officer from 1987 through 1993.

During his tenure at REN he led the company from a start-up venture to a position as the fourth largest dialysis provider in the United States with revenues exceeding $100,000,000 per year. During that time the company was taken through an IPO and sold to a strategic acquirer (Gambro AB) for an implied value of $400,000,000.

In 1993 he founded Medical Information Management Systems, Inc., a medical software company (MIMS). MIMS was a pioneer in the development of electronic medical records and was the first company to have an Internet accessible comprehensive medical record solution. MIMS received funding from a number of sources including Bristol-Myers-Squibb. It was ultimately sold to National Nephrology Associates.

In 1998 Dr. Tannenbaum became Chairman and Chief Executive Officer of National Nephrology Associates, Inc., a company he co-founded with some of his original management team from REN. Following an investment from J.W. Childs and Indosuez Capital, Dr. Tannenbaum led NNA from a start-up venture to become the fifth largest dialysis provider in the United States with 85 clinics operating in 14 states and annual revenues that exceeded $250,000,000.

In March 2004, Renal Care Group, a New York Stock Exchange Company, purchased NNA for $345,000,000. Dr. Tannenbaum left NNA in June 2003 to co-found DSI with Dr. G. Patrick Maxwell and certain members of the NNA, MIMS, and REN former management teams.

G. Patrick Maxwell, M.D.
Executive Vice President - Chief Surgical Officer
Dr. Maxwell is board certified in plastic surgery by the American Board of Plastic Surgery and has gained international recognition as one of the premier plastic surgeons in the world. He received his medical degree in 1972 from Vanderbilt University in Nashville, Tennessee, and performed his residencies in general and plastic surgery at The Johns Hopkins Hospital in Baltimore, Maryland.

He completed a fellowship in microsurgery at the University of California, San Francisco, and a fellowship in hand surgery at the Curtis Hand Center in Baltimore, Maryland. He is a member of the clinical faculty at the Department of Plastic Surgery at Vanderbilt. In 1993, Dr. Maxwell served as nationwide visiting professor for the Plastic Surgery Education Foundation.

Dr. Maxwell is internationally recognized for his innovative surgical techniques, particularly in the field of breast reconstruction and cosmetic surgery of the face and body. He has operated in approximately 20 different countries and is a member of several plastic surgery societies, including those in the United States, Canada, Japan and South Africa.

He pioneered ultrasonic liposuction in the United States and is an authority on body contouring. He developed the first textured anatomical tissue expander for breast reconstruction and designed the anatomical implants for cosmetic and reconstructive breast surgery.

Dr. Maxwell has received numerous awards in plastic surgery.

Additionally, in 1991 he was selected by his peers to be included in The Best Doctors in America and in every subsequent “Best Doctors” list published from then to the present.

In 2000, Town and Country magazine ranked him among the top plastic surgeons in the United States and in 2001, W magazine named him to its list of the world’s best plastic surgeons. He has lectured and demonstrated cosmetic surgical procedures worldwide, holds patents for products and instruments used in cosmetic surgery and has published numerous articles on cosmetic and reconstructive surgical procedures. He is considered an authority on cosmetic and reconstructive breast surgery and is highly respected for facial aesthetic cosmetic surgery. He performs procedures on patients who travel from all over the world for his artistic approach.

Dr. Maxwell founded the Institute for Aesthetic and Reconstructive Surgery at Baptist Hospital in Nashville, Tennessee, where he has trained post graduate plastic surgery fellows and Vanderbilt University plastic surgery residents for the last 15 years, as well as serving as Clinical Director of the Institute. He is cofounder of the Integrative Aesthetics Skin Care Company and Clinical Director of the Inamed Academy, which is based in Santa Barbara, California.

M. Stephen Harrison
Executive Vice President - Chief Development Officer

Mr. Harrison is a 35 year veteran of the healthcare services industry. He was responsible for founding Community Dialysis Clinics in the mid-1970s, which ultimately was transformed into Vivra, a New York Stock Exchnge company, and finally acquired by Gambro. While at Vivra, the company grew from 4 dialysis clinics to 36 clinics.

In the early 1980s he formed a privately held dialysis company, and in 1990 came to REN Corporation USA as the head of new business development.

At REN, he grew the company from 15 dialysis clinics to over 70 clinics. In 1998 he was a co-founder with Dr. Tannenbaum of National Nephrology Associates. While there, he grew the company from startup, with no dialysis clinics to 85 clinics.

Over the years Mr. Harrison has spearheaded the development of over 200 alternate site therapy locations, mostly in the areas of chronic and acute dialysis. His responsibilities included medical director negotiation, site selection, construction management, provider-payer relations, and licensing and regulatory.

Judy W. Lefkovitz
Executive Vice President - Chief Administrative Officer

Mrs. Lefkovitz, who received a Bachelor of Science degree from Tulane University and a Master of Science degree from The University of Tennessee in Knoxville, has over 25 years experience in business systems and data processing.

Most recently, she was executive vice president and general manager for IMRAC Corporation, an IT affiliate of National Nephrology Associates.

At IMRAC, she oversaw operations, human resources, sales, marketing, customer service, contract billing services, HIPAA compliance and program development. While there, she was responsible for information technology functions used by all the facilities owned or operated by National Nephrology Associates.

These systems include a fully integrated electronic medical record, a physician order entry system, centralized perpetual inventory control, centralized risk management, laboratory systems for each facility, centralized asset tracking, automated drug dispensing, centralized automated purchasing, centralized payable processing, centralized payroll processing and employee benefits management, centralized billing and collections, multi-facility and multi-department general ledger, clinical quality assurance and automated staff assignment scheduling.

National Nephrology’s home office systems managed the procurement and delivery of approximately 25,000,000 supply items per year for almost 1,000,000 outpatient dialysis visits and 40,000 in hospital, critical care unit dialysis treatments per year.

In addition, these systems managed the collection and government reporting of the quality assurance and medical data on each of these treatments, the procurement and administration of over 4 million intravenous drug infusions per year, and the procurement and reporting, including critical value alerting, of over 7,000,000 laboratory studies per year. The registration, discharge, and transfer activities involving these patients, and interaction with the representatives of various managed care entities who contracted for services with NNA, nationwide, were under the general direction of Mrs. Lefkovitz.

Jay A. Yalowitz
Executive Vice President – General Counsel

Prior to joining DSI Holding Company as General Counsel, Mr. Yalowitz was in private practice in Chicago for almost 15 years, as a partner with McGuireWoods and prior to that, Ross & Hardies.

Mr. Yalowitz represented health care providers including dialysis facilities, hospitals, ambulatory surgery centers, physician practices and ancillary providers in a wide range of matters including mergers and acquisitions, regulatory matters and corporate matters.

Mr. Yalowitz received his undergraduate degree from the University of Michigan and his law degree from Washington University in St. Louis.

Brenda Kurnik, M.D., FACP
Senior Vice President - Business Development

Dr. Kurnik   received her undergraduate degree from the Massachusetts Institute of Technology and her M.D. from Washington University School of Medicine in St. Louis.

She completed her residency training at Upstate Medical Center, Syracuse, NY.  She subsequently completed her nephrology training at Washington University/Barnes Hospital, after which she joined the faculty of Robert Wood Johnson Medical School/ Cooper University Hospital.

While at Robert Wood Johnson she was appointed Division Head of Nephrology in 1994, and Associate Chief of Medicine in 1999. As Division Head she increased the divisional revenue ten fold from under $300,000 in 1993-1994 to $3,000,000 in 2000-2001. Departmental revenue increased from $34,000,000 to $51,000,000 during her leadership between 1998 and 2001.

She played an instrumental role in expanding services, opening satellite offices and dialysis units and faculty recruitment. She was Chairman and organizer of an outreach conference on Preventive Health, successfully attracting  keynote speakers Dr Benjamin Spock and Dr Linus Pauling.

Dr. Kurnik was Vice President and President Elect of the Medical Staff prior to leaving to become Vice Chair of Medicine at Drexel University College of Medicine in 2001. Subsequently, she was appointed Director of   Business Development for the Department of   Medicine.  At Drexel she   was also very successful in developing   business and   recruiting   faculty.

She was elected as the medical school representative to the Drexel University Budget, Planning and Development committee   In 2002 she was accepted into and completed the Harvard School of Public Health two week intensive course “Leadership Development for Physicians in Academic Health Centers”.

Dr. Kurnik has also made significant contributions in both bench research and subsequently in clinical research, achieving an international reputation in the area of radio-contrast induced acute renal failure. 

Dr. Kurnik is a former President of the New Jersey Nephrology Society. She also served for several years as a member of the national US Healthcare/Aetna nephrology quality assurance committee and was listed as a Philadelphia magazine “Top Doc” in 1994, 1996, 1999 and 2002.
